Privacy policy

Spine Health & Wellness (“we”, “us”, “our”) runs this website, spinehealthandwellness.com, and six chiropractic and rehabilitation clinics in North Texas. This policy explains what information the website collects, how we use it, and the choices you have.

Your health information as a patient

If you are a patient, our Notice of Privacy Practices explains how we use and protect your health information under HIPAA. That notice is separate from this website policy. You can ask for a copy at any of our clinics or by calling (972) 556-5667.

Information you give us

Appointment requests. Our booking form asks for your first and last name, email address and mobile phone number, and lets you add an optional note and choose a date and time. You can choose whether to receive emails and text messages from us. The form includes a short “I’m not a robot” check to block spam. The form is provided by Review Wave, the scheduling service we use.

New patient intake paperwork. Our patient intake page is part of becoming a patient, not a general website form. It asks for the details we need to treat you and bill for your care, such as your contact details, date of birth, Social Security number, driver’s license, employer, insurance information, a description of your symptoms and your signature. It is sent over an encrypted connection to our website system for our staff to review. If you would rather not complete it online, call us at (972) 556-5667.

Online payments. When you pay a bill, you do so on a secure payment page run by Stripe. Stripe collects the amount, the patient’s name, an optional statement number, your card or wallet details and an email address for your receipt. We never see your full card number.

Calls and emails. If you call or email us, we receive whatever you choose to share.

Information collected automatically

Like most websites, our web server records basic technical information when you visit, such as your IP address, browser type, the pages you request and the date and time. Security tools on our website use this information to block abuse.

We also use cookies and similar technologies, which are small files or entries stored in your browser:

• Google Analytics shows us how visitors find and use our website, such as which pages are viewed.
• Google Ads measures whether our ads lead to visits and appointment requests, and may be used to show our ads to people who have visited our website.
• Meta Pixel (Facebook and Instagram) measures page views and appointment requests from our ads, and may be used to show our ads on Facebook and Instagram.
• Our booking tools store small entries in your browser so the booking form works and so a completed request is counted once. When a request is completed, we tell Google and Meta that a booking happened. We do not send them what you typed into the form.
• Our patient intake page may set cookies needed for the page to work, to keep it secure and to note how you arrived at our site.

Content from other companies

Some pages show content from other companies. When that content loads, the company receives your IP address and basic browser information and handles it under its own privacy policy.

• Google Maps on our clinic and area pages
• YouTube patient story videos, shown in YouTube’s privacy-enhanced mode
• Google Fonts, which provides the lettering on our pages
• Review Wave, which shows our patient reviews and runs our booking form

Our website also links to other sites, such as our Facebook and Instagram pages and the research cited in our articles. Their privacy policies apply once you leave our website.

How we use information

Who we share information with

We share information only with service providers that help us run our website and our practice, and only for the purposes above:

• Cloudways, which hosts our website
• Review Wave, for appointment requests, reminders, emails and text messages
• Stripe, for online payments
• Google, for analytics, advertising, maps, videos and fonts
• Meta, for advertising on Facebook and Instagram
• Our email providers, to send and receive messages
• Companies that manage our website and advertising for us, under our instructions

We may also share information when the law requires it, or to protect the rights, safety and property of our patients, our staff or others.

We do not sell your personal information.

Your choices

• You can block or delete cookies in your browser settings. Some parts of the website, such as the booking form, may not work without them.
• You can stop Google Analytics from recording your visits with the Google Analytics opt-out browser add-on.
• You can unsubscribe from our emails at any time using the link in the email, and stop text messages by replying STOP.
• You can ask us to update or delete information you sent through this website by contacting us below.

How long we keep information

We keep information for as long as we need it for the purposes above, or as long as the law requires.

Security

Our website uses an encrypted (HTTPS) connection, and we limit access to information to the people and service providers who need it. No website or online service is completely secure, so please do not send sensitive health details through our website forms.

Children

Our website is not directed to children under 13, and we do not knowingly collect personal information online from children under 13. If you believe a child has sent us information through this website, please contact us and we will delete it.

Changes to this policy

We may update this policy from time to time. When we do, we will post the new version here and change the date at the top of this page.
